Fok’s Insurance Agency, Fok’s Financial
Services, 1216251 Ontario Inc., we are in compliance
with Canada’s New Privacy Law that comes on
January 1st, 2004, the Personal Information Protection
and Electronic Act (PIPEDA). We understand and respect
our customers’ right to privacy.
When
you contact us by mail, email, phone, fax, in person,
website etc. any information you give us is kept strictly
confidential and is not used for any purpose other
than for helping you buy the insurance plan through
us. After you have purchased the insurance through
us, the information that you provided to us may be used for customer service through us, through the
insurance carriers, through the service providers,
and through the underwriters. Your information will only be used for the purposes of responding to and processing your requests for insurance and financial
services. However, it may be used by us to call/email/mail/fax
you for follow-up, expiration notices, renewal notices,
plan updates and periodic customer contact, such as
birthday card or Christmas card mailings.
Your choice to use our Website will be considered your
consent for us to use the information collected therefrom
as stated in this statement.
We
restrict access to your personal information only
to those employees who need that information in order
to provide services to you.
We will not share, give, provide or otherwise disseminate
any information that would identify you personally, such as address, phone number, credit card number,
email address etc. to anyone, other than the companies
that will service your policy.

we collect, use and disclose your personal information

the suitability of our products and services for

beneficiaries etc. that are required by the insurance
carriers;
- Assist us to understand the current and future needs of our customers, and to provide ongoing services.

for as long as it is needed to fulfill the identified
purposes, or as required or permitted by law.

will make all reasonable efforts to ensure that any
personal information we collect and keep is as accurate,
complete and as up-to-date as required for the identified
purposes. To do so, we will rely to a large extent
on you to provide us with accurate information and
to inform us of changes, such as changes in your contact
information.
